A visita médica é um exame médico obrigatório exigido pelos serviços de imigração dos EUA. Consiste em um exame físico e exame do seu histórico médico. Preparação para o exame médico

Você deve ir ao nosso centro de radiologia e laboratórios designados - caso contrário, seu histórico médico não será validado. Os resultados de outros centros não serão aceitos.

1. Atualizar vacinas

Você deve consultar o seu médico de família o mais rápido possível para atualizar suas vacinas de acordo com a nossa lista de vacinas aqui (diretrizes dos EUA):

2. Marcar consulta de raio-x

Se tiver 15 anos ou mais, você deve marcar uma consulta para uma radiografia de tórax por pelo menos uma hora NTES sua consulta médica em nosso centro designado.

3. Testes laboratoriasis

Você deve realizar testes laboratoriais ANTES a visita médica para sífilis e gonorréia (caso tenha idade igual ou superior a 15 anos) apenas em um destes centros: Laboratoire Clément ou Bio-Path. Você não precisa estar em jejum para os exames de sangue, mas não deve urinar por 2 horas antes do exame de urina. Nesses laboratórios, você não precisa fazer uma consulta ou trazer uma receita. Você deve trazer seu passaporte e pagar seus testes diretamente.

Vacinas

Sou um adulto de 19 anos ou mais. Quais vacinas eu preciso?

  1. Difteria, tétano, tosse convulsa e poliomielite
    Você deveria ter tido a vacinação primária tripla com DTaP e Polio. Você também deverá ter tido um reforço DT (difteria e tétano) nos últimos 10 anos. Se você nunca teve uma vacina pertussis (tosse convulsa), você necessitará realizar agora a vacinação combinada de DTap e Polio. A vacina contra tosse Whooping individual já não existe mais.
    Informações importantes sobre a vacinação contra a pólio-devido à presença de vírus da pólio selvagem em determinados países, a autoridade de imigração dos Estados Unidos exige agora um Booster de vacinação contra a pólio nas últimas 4-52 semanas, se você viver ou tiver passado mais de 4 semanas em Papua Nova Guiné, Indonésia, Mianmar, Somália, Paquistão, Afeganistão e Nigéria.
  2. Sarampo, caxumba e rubéola
    Se você nasceu em 1957 ou mais tarde, você deverá ter tido uma dose de MMR. Se você não tem uma prova escrita de vacinação, você pode obter a vacinação de MMR, ou, obter um exame de sangue para mostrar imunidade a todas as três infecções (níveis de sorologia). Se os resultados do seu sangue apenas mostram imunidade a uma ou duas das 3 infecções, você precisará de uma vacinação MMR completa. Não existem vacinas individuais contra as 3 infecções. NB: você não será obrigado a tomar a vacina MMR se estiver grávida, pois é uma vacina viva.
  3. Varicela (varíola)
    Se você nunca teve varíola, você precisará iniciar um curso de 2 vacinas, com 1 ano de intervalo, ou realizar um exame de sangue para ver se você é imune.
    Você não precisará de vacinação se você teve varíola quando criança (ou adulto). Não precisamos de uma prova escrita de você ter tido a doença.
    NB: Você não será obrigado a tomar a vacina Varicela se estiver grávida, pois é uma vacina viva.
  4. Vacina anual contra influenza sazonal
    Apenas disponível / exigido se a consulta de visto ocorrer durante a temporada de gripe (Normalmente de Setembro até o início de Fevereiro). Se ainda estiver disponível nas farmácias no momento da visita médica, você deve ser vacinado. Verifique com seu farmacêutico local. 2020 A vacina contra influenza sazonal está disponível na França a partir de 13 de outubro.
  5. Pneumococo
    A vacinação pneumocócica é necessária a partir dos 65 anos de idade. Para aqueles que nunca tiveram a vacinação, o Centro Americano de Controle de Doenças (CDC) recomendou uma dose inicial de P13 seguida de, pelo menos um ano depois, por PPSV23.

Quem deve realizar minhas vacinas?

O que posso trazer à visita médica como prova das minhas vacinas?

E se eu chegar na visita médica e não tiver recebido todas as vacinas?

Se você não está atualizado com suas vacinas quando tiver a sua consulta com o Dr Jolly ou Dr Gunoy, você precisará realizer e pagar um novo agendamento para receber as vacinas. Tenha em mente que, para esse segundo agendamento, podemos não termos dias/horários disponíveis antes do seu compromisso da Embaixada dos EUA.

E se eu recusar certas vacinas?

Dr Jolly e Dr Gunoy não irão obrigá-lo a tomar certas vacinas se não estiver interessado nelas, ou se quiser entrar em contato com os Serviços de Cidadania e Imigração dos EUA (ver website) para obter informações de isenção (www.uscis.gov). Os candidatos devem ser vacinados até a data com os requisitos do CDC americano para que possam requerer o visto.


A visita médica

O que levar ao exame médico (para cada candidato)?

  1. Um passaporte válido
  2. O número do seu processo / agendamento com a Embaixada
  3. Certificados de vacinação concluídos (ou resultados de sorologia) de acordo com as diretrizes americanas do CDC.
  4. CD de computador de raio-x de tórax (se completado) do centro de radiologia EIFFEL Levallois-Perret.
  5. Informações sobre o seu estado médico (histórico médico), passado e presente (prescrições, detalhes de doença grave ou cirurgia).
  6. Pagamento pela visita médica - dinheiro, cheque, cartão bancário.

Após a visita

Quando recebo o arquivo médico completo?

Se você completar os exames e as vacinas necessários antes da visita do médico, você receberá seu arquivo completo imediatamente. Se você quiser voltar para o consultório do médico 24 horas após a consulta do médico, em uma visita curta de apenas 5 minutos, poderemos enviar os resultados diretamente para a Embaixada. Se você optar por não coletar o arquivo médico, traga um envelope Chronopost de 2kg (envelope 28 euros) para enviar resultados para a Embaixada.

Quanto tempo o arquivo médico completo permanece válido?

Seis meses se o exame for normal. Se você não recebeu seu visto no final de seis meses, você será obrigado a repetir e pagar a visita médica, exames de laboratório e radiografia de tórax.

Como fazer os exames médicos nos EUA e visitar tudo em um dia

Introdução

NOVAS DIRETRIZES SIMPLES PARA A PREPARAÇÃO PARA O EXAME MÉDICO

Médicos designados pelos serviços dos Estados Unidos, Doutor Jolly e Doutor Gunoy, realizam os exames médicos exigidos pelos serviços de Imigração dos Estados Unidos como parte de um pedido de visto e/ou imigração. Esperamos que seu exame médico seja o mais fácil e sem estresse possível.

O exame médico é um exame obrigatório solicitado pelos serviços de imigração americanos que envolve um check-up físico e o exame de seu histórico médico. O exame de 30 minutos ocorre em nosso consultório em 7 rue de la Pompe, 75116 Paris. A estação de metrô mais próxima é La Muette, a 3 minutos a pé. 1. Covid-19 Trazer 2 papéis impressos com as 2 datas de vacinação contra a Covid 19. Não aceita QR code. Seu arquivo será bloqueado sem comprovação das 2 datas.

2. Difteria, Tétano, Coqueluche e Poliomielite Para fazer se não houver recall por 10 anos.

4. Varicela Vacina contra-indicada em caso de gravidez Se você nunca teve varicela, deve se vacinar. Se preferir, pode fazer um exame de sangue para ver se está imune. Peça uma receita ao seu médico. Se você não está imune, você deve se vacinar novamente. Uma história de varicela será suficiente apenas se você tiver a data confirmada por um médico.

5. HEPATITE B até 59 anos inclusive. Série de 3 vacinações a serem concluídas após a entrevista, se você não tiver tempo para fazê-las todas.

6. Influenza (gripe sazonal) A vacina é obrigatória durante a temporada de gripe (início de outubro a final de janeiro, desde que a vacina esteja disponível nas farmácias). Verifique a disponibilidade com o seu farmacêutico.

7. Pneumocócica A vacina pneumocócica é obrigatória a partir dos 65 anos. O Centro Americano de Controle de Doenças (CDC) solicita uma primeira vacinação com a vacina pneumocócica PPSV23

Exames laboratoriais (se tiver entre 18 e 44 anos)
Se tiver entre 18 e 44 anos será encaminhado para a realização dos seus exames laboratoriais após a consulta médica. Sem necessidade de agendamento. Você não precisa estar em jejum para os exames de sangue, mas não deve urinar por 2 horas antes do exame de urina para gonorreia (se aplicável). Você será enviado pelo médico para um de nossos laboratórios designados abaixo. Não vá antes da visita:

Após a visita

Quando a Embaixada receberá o arquivo médico completo?
Todos os arquivos são enviados eletronicamente (instantaneamente) para a embaixada assim que estiverem completos. Isso pode, portanto, ser dentro de 1 dia. Para completar o arquivo, precisamos de suas vacinas em dia e da radiografia de tórax e resultados laboratoriais, se aplicável. Isso NÃO afeta sua consulta na Embaixada e você ainda pode ir à consulta na Embaixada sem o relatório médico. Se, no entanto, você quiser que seu arquivo médico esteja pronto para o dia da sua consulta na Embaixada, você deve comparecer ao seu exame médico pelo menos duas semanas antes da sua entrevista na Embaixada.

US VISA - Medical Visit Information

Introduction

Dr Jolly and Dr Gunoy are designated physicians for the US Immigration Services who carry out the medical examination for visa applications and immigration. We are designated for those whose visa exam is in Paris or are from Portugal. As an emergency measure we are permitted to carry out the visa medical visit for Ukrainians whose embassy visit is in Frankfurt, Germany.  We are not designated for other nationals whose visa appointment is in Frankfurt. We hope that your medical exam is as stress-free and uncomplicated as possible.

The medical visit is an obligatory examination requested by US Immigration services involving a physical check-up and examination of your medical history. The 30-minute exam takes place in our consulting room at 7 rue de la Pompe, 75116 Paris. The nearest metro is La Muette, a 3-minute walk away. Depending on your age the exam will be followed by a chest x-ray (15 years and older) and laboratory tests (18-44 years old) at our designated centres 5 minutes away (see below).

How to book an appointment:

  • Please send us your US file umber if you are K visa, NIV or Diversity. Either give it to the secretary when you book or send it by email.
  • Do not register your American file on the IVMIS booking site until you have an appointment confirmed via Doctolib or the secretary.

It is very important to be vaccinated in line with US regulations. You can start updating your vaccinations as soon as possible. This is explained below.


What you must do before the medical

1. UPDATE YOUR VACCINATIONS

We will need to see printed proof of 2 DATES of COVID-19 vaccination from 5 years of age or older. Both dates are important (or only 1 in the case of Janssen). We cannot use your QR code

Delays in completing medical reports are usually due to us waiting for you to complete your vaccinations. To avoid this please go to see your family doctor as soon as possible to update your vaccinations in line with our list of vaccinations below (following US guidelines). If you do not have a doctor you will have to wait until your medical appointment to discuss the missing vacccines with Dr Jolly or Dr Gunoy.

If you do not bring proof of all vaccinations or blood tests showing immunity your file will not be completed immediately. Both the doctor and the Embassy will wait for you to send the remaining proof to complete the file. You can still go to your Embassy interview while waiting to complete the medical file.

2. You will carry out a chest X-ray (from 15 years old) and laboratory tests (18-44 years old) after the medical. 

PREGNANT WOMEN MUST BOOK A CHEST X-RAY APPOINTMENT FOR 1 HOUR AFTER THE MEDICAL VISIT. ALL OTHERS CAN CHOOSE TO EITHER BOOK AN APPOINTMENT OR WALK-IN WITHOUT AN APPOINTMENT TO THE X-RAY CENTRE. THE CHEST X-RAY MUST TAKE PLACE AFTER THE MEDICAL VISIT WITH A PRESCRIPTION PROVIDED BY OUR DOCTORS;

If pregnant you must book an appointment for a chest x-ray to take place after your medical. We only accept chest x-rays with Dr. Frédéric Zeitoun or Dr. Daniel Szwarc from the designated clinic below. In the case of pregnancy, prior consent must be signed with Dr. Jolly or Dr. Gunoy prior to the X-ray.

* If you have ever had a criminal conviction relating to drugs or alcohol the doctor is required to prescribe specific tests for you during the medical.

* If you are pregnant or suspect you might be pregnant the doctor will discuss the chest x-ray with you during the visit and you will be required to sign a consent form.

What to bring to the medical examination (for each applicant)

  • A valid passport. If the embassy has it then ask us for a copy well in advance. It is best to always keep a copy of your passport.
  • Your Embassy appointment letter containing your case number
  • Immigrant visa - your DS 260 confirmation form
  • Completed vaccination certificates (or serology results) in line with American CDC guidelines.
  • COVID 19 vaccination, 2 dates. You must bring a printed version of each of your 2 Covid vaccination certificates or only 1 in the case of Janssen.
  • Information about your medical status, past and present (prescriptions, details of major illness or surgery).
  • Cheque, cash or credit card for payment of medical visit.

How do I cancel or change my medical appointment?

Following the visit

The doctor will send you with prescriptions to carry out your chest x-ray and laboratory tests. You must not go before the medical visit.

Laboratory tests (if aged 18-44 years old)

If aged 18-44 years you will be sent to to carry out your laboratory tests after the medical visit.  No appointment needed. You do not need to be fasting for the blood tests but should not urinate for 2 hours before the urine test for gonorrhoea (if applicable). You will be sent by the doctor to one of our designated laboratories below:

When will the Embassy receive the completed medical file?

All files are sent electronically (instantly) to the embassy as soon as they are complete. This can therefore be within 1 day. To complete the file we need your vaccinations up to date and the chest x-ray and laboratory results if applicable.  It does NOT affect your Embassy appointment and you can still go to the Embassy appointment without the medical report. The Embassy may take 2 weeks to access and review your medical file. If  you would like your medical file to be ready for the day of your Embassy appointment, you should come for your medical examination at least 2 weeks before your interview at the Embassy.

How long does the completed medical file remain valid?

Six months if the examination is normal. If you have not received your visa by the end of six months, you will be obliged to repeat and pay for the medical visit, laboratory tests and chest x-ray.

Adult Vaccinations: compulsory if aged 19 years or older

(For anyone under 19, baby, child or teenager, see Child Vaccinations)

ALL vaccines on this list are required by the American government to obtain the US visa. No other vaccinations will be asked of adults. Bring your vaccination records to the medical visit. Please bring written proof of vaccination during childhood (if you have it).
 
COVID-19
We need to record the first 2 vaccination dates. You must show 2 printed sheets (not the QR codes), one for each date, and give a copy to the doctor.
 
Diphtheria, Tetanus, Pertussis (DTaP) & Poliomyelitis
To get a booster if you have not had one in the past 10 years. If you have ever been vaccinated against pertussis the booster does not need to include it.
POLIO special case - you need to have had a polio vaccination booster within the last 4-52 weeks if you live or have spent more than 4 weeks in certain countries designated as at high risk. You must consult the list of all current countries affected on the CDC page here: Polio vaccination
 
Rubella, Mumps, Measles (MMR)
To do if you were born after 1957. If you do not have written proof of vaccination, you must get vaccinated, or if you prefer, take a blood test to see if you are immune to the 3 diseases. Ask your doctor for a prescription. If you are only immunized against 1 or 2 of these diseases, you must be vaccinated again with MMR. Individual vaccines do not exist. A history of Rubella, Mumps, or Measles disease is not sufficient as proof of immunity
If you are pregnant, you will not be asked to be vaccinated with the complete MMR because it is a “live” vaccine.
 
Hepatitis B
All those aged 0 to 59 years must be vaccinated against hepatitis B

Varicella
If you have never had chickenpox disease you should get vaccinated. If you prefer you can take a blood test to see if you are immune. Ask your doctor for a prescription. We do not need written proof of your history of the disease but the approximate date. If you are pregnant, you will not be asked to get vaccinated against chickenpox because it is a “live” vaccine.
 
Pneumococcal
The Pneumococcal vaccine is compulsory from the age of 65. The US Center for Disease Control (CDC) requests a first vaccination with the Pneumococcal PPSV23 vaccine
 
Influenza (seasonal flu vaccine)
The vaccine is required if your medical exam takes place during the flu season (early October to February or as long as the vaccine remains available in pharmacies). Check availability with your pharmacist.

Frequently asked questions

Who should carry out my vaccinations?

Please see your family doctor in advance to obtain the required vaccinations. Your child may see a paediatrician. If you book your medical at least two weeks in advance, we consider that you have time to see your doctor.

What can I bring to the medical visit as proof of my vaccinations?

You can bring your carnet de santé (if you have one) or signed vaccination certificates/cards of all previously completed vaccinations. A signed letter from your family doctor listing your vaccinations and the dates they were given will also be accepted. Each vaccination must be dated and signed individually by the doctor. You can also bring blood test results to prove your immunity (for measles, mumps and rubella). Your doctor can prescribe the blood tests but bear in mind that the results make take at least a week to arrive.

What if I arrive at the medical visit and have not completed my vaccinations, nor written to you in advance asking for a prescription?

If you are not up to date with your vaccinations on the day of your appointment with Dr Jolly or Dr Gunoy, you will need to make and pay for a new appointment to receive relevant vaccinations. Bear in mind this second appointment may not be available before your USA Embassy appointment.

What if I refuse certain vaccinations?

Dr Jolly and Dr Gunoy will not oblige you to take certain vaccines if you are opposed to them on moral or religious grounds, but you will have to contact the U.S. Citizenship and Immigration Services (see website www.uscis.gov for waiver information). Applicants must be vaccinated to date in line with the American CDC requirements for visa applicants.

The medical visit is an obligatory requirement by US Immigration services involving a physical check-up and examination of your medical history.

Please read this information before booking an appointment. In particular, you will need to have been vaccinated in line with US regulations. And if you are 15 years or older you must also have a chest X-Ray and laboratory tests for syphilis and gonorrhoea only at one of the designated facilities.

NEW for Immigrant status applicants only - According to a new US Immigration requirement all panel physicians must use the electronic healthcase system eMedical. Once you have booked an appointment with us we check to see if you are on eMedical. We will then send an email to all immigrant status applicants to say if you need to carry out the chest x-ray and lab tests BEFORE OR AFTER the medical visit.

All other visa cases (Diversity, K-visa or Non-immigrant visa..) are not affected by eMedical, so they must carry out the chest x-ray and lab tests BEFORE the medical visit and they will not receive an email about the timing of the x-ray and tests.

The 20-30-minute exam takes place in our consulting room at 7 rue de la Pompe, 75116 Paris. The nearest metro is La Muette, a 3-minute walk away.

We can generally provide an appointment within two weeks, but sometimes it can be fully booked several weeks in advance. We use the online booking service IVMIS (see below). Preparing for the Medical Visit

Immigrant status cases will receive an email to say if you need to carry out the chest x-ray and lab tests BEFORE OR AFTER the medical visit, so do not book the x-ray appointment or do laboratory tests before we advise you.

All other cases (Diversity, K-visa or Non-immigrant visa..)  MUST carry out the chest x-ray and lab tests BEFORE the medical visit.

      1. Answer all the questions and select an appointment. Appointments take place only on the hour and half hour e.g. 10.00 a.m., 10.30 a.m. 11.00 a.m.
      2. You will arrive at a page with a pink sign saying “Your appointment is not yet created! Please review the information below and click "Make Appointment" to create your appointment”. Check that all the information on the page is correct, and then click on Make appointment at the bottom of the page.
      3. We will check your information and send you an appointment number. Please note that sometimes the confirmation email may arrive in your spam folder. Important: If you do not receive a confirmation with an appointment number you do not have an appointment. You must start again.

Following the visit

When will I receive the completed medical file?

If you complete all the necessary tests and vaccinations before the Doctor’s visit, you will receive your completed file immediately. If not, you will have to come back yourself to pick up the medical file 24 hours after the Doctor’s appointment (this will take 5 minutes) We can alternatively send the results directly to the Embassy. If you are not collecting the medical file yourself, please bring a 2kg Chronopost envelope (padded envelope about 28 Euros) to send results to the Embassy.

In eMedical visa cases the file will be sent directly to the immigration services electronically. The eMedical procedure sometimes takes a day or 2 longer to gather results and send the report to the Embassy. This does NOT affect your Embassy appointment. You can still go to the Embassy appointment without the medical report as they are aware of this delay and it takes them up to 14 days to process your visa application. Do not write to us about the transfer of the emedical file as it is sent automatically and we will always contact you if there is a problem with your results.

You should have had one dose of MMR if you were born in 1957 or later.

If you never had an MMR vaccination you will need one, or, if you prefer, you can have a blood test first to see if you are immune to all three infections (serology levels). Ask your doctor for a prescription. If your blood results only show immunity to one or two of the 3 then you will need a full MMR vaccination. Individual vaccines against the 3 infections do not exist.

A history of MMR without blood test results is not proof of immunity>

You will not be required to take the MMR vaccine if you are pregnant as it is a live vaccine.

Varicella (Chicken pox)

Varilrix®, Varivax®

If you had the illness chicken pox as a child (or adult) you do not need vaccination (no written proof needed).

If you never had it you will need a vaccination, or, if you prefer, you can have a blood test first to see if you are immune. Ask your doctor for a prescription.

You will not be required to take the Varicella vaccine if you are pregnant as it is a live vaccine.

Yearly seasonal influenza vaccine

Vaxigriptetra, Fluarixtetra, Influvac Tetra

Required if visa appointment is during the flu season (beginning of October to the beginning of February. Check with your local pharmacist to see if it is available at the time of your medical visit. 2019 Vaccinations started October 15th

Introduction

Dr Jolly and Dr Gunoy are designated physicians for the US Immigration Services. They carry out the medical examination for visa applications and immigration. The medical visit is an obligatory examination required by US Immigration services involving a physical check-up and examination of your medical history. The 30-minute exam takes place in our consulting room at 7 rue de la Pompe, 75116 Paris. The nearest metro is La Muette, a 3-minute walk away.

You have various options for booking an appointment:

Please read the information below before booking an appointment. In particular, you will need to have been vaccinated in line with US regulations. If you are 15 years or older you must also have a chest X-Ray and laboratory tests for syphilis and gonorrhoea. You must carry them out  BEFORE the medical visit and only at one of the designated centres listed below.

Be vaccinated in line with US guidelines

No appointment needed. Contact us for your prescription. Do not urinate at least 2 hours before the tests. You can eat and drink before the tests

Carry out a chest x-ray if aged 15 or over before the medical. It is recommended to book an appointment but if this is not possible you will be seen without one.

What to bring to the medical examination (for each applicant)

  • A valid passport
  • Your Pre-visit Checklist duly completed
  • Your Embassy appointment letter containing your case number
  • Completed vaccination certificates (or serology results) in line with American CDC guidelines.
  • Computer disc of your chest x-ray.
  • Information about your medical status, past and present (prescriptions, details of major illness or surgery).
  • Cheque, cash or credit card for payment of medical visit.

We use an Internet booking service to make appointments. Please use this button to Make an appointment

      1. Answer all the questions and select an appointment. Appointments take place only on the hour and half hour e.g. 10.00 a.m., 10.30 a.m. 11.00 a.m.
      2. You will arrive at a page with a pink sign saying “Your appointment is not yet created! Please review the information below and click "Make Appointment" to create your appointment”. Check that all the information on the page is correct, and then click on Make appointment at the bottom of the page.
      3. We will check your information and send you an appointment number. Please note that sometimes the confirmation email may arrive in your spam folder. Important: If you do not receive a confirmation with an appointment number you do not have an appointment. You must start again.

There are no appointments available before my US Embassy visit. What can I do?

How do I cancel or change my medical appointment?

You should cancel, check or change your appointment yourself (using your appointment number and passport number) via the following link: Cancel/Change Appointment

In the case of a last-minute cancellation (less than 48 hours beforehand), you might be asked to pay a cancellation fee.

Following the visit

When will I receive the completed medical file?

If you complete all the necessary tests and vaccinations before the Doctor’s visit, you will receive your completed file immediately. If not, you will have to come back yourself to pick up the medical file 24 hours after the Doctor’s appointment (this will take 5 minutes) We can alternatively send the results directly to the Embassy. If you are not collecting the medical file yourself, please bring a 2kg Chronopost envelope (padded envelope about 25 Euros) to send results to the Embassy.

How long does the completed medical file remain valid?

Six months if the examination is normal. If you have not received your visa by the end of six months, you will be obliged to repeat and pay for the medical visit, laboratory tests and chest x-ray.

Diphtheria, Tetanus, Whooping cough (DTaP) & Polio

Boosterix, Repevax

Get diphtheria, tetanus and polio booster if you have not had one within the last 10 years. The booster should also include whooping-cough if you have never had that vaccination.

Bring the dates of your childhood triple primary vaccination with DTaP and Polio if you have the written proof.

If you live in or have spent more than 4 weeks in Papua New Guinea, Indonesia, Myanmar, Somalia, Pakistan, Afghanistan or Nigeria you must have had a polio vaccination booster within the past 4-52 weeks.

Measles, Mumps and Rubella (MMR)

MMRVaxPro®, Priorix®

You should have had one dose of MMR if you were born in 1957 or later.

If you never had an MMR vaccination you will need one, or, if you prefer, you can have a blood test first to see if you are immune to all three infections (serology levels). Ask your doctor for a prescription. If your blood results only show immunity to one or two of the 3 then you will need a full MMR vaccination. Individual vaccines against the 3 infections do not exist.

A history of MMR without blood test results is not proof of immunity>

You will not be required to take the MMR vaccine if you are pregnant as it is a live vaccine.

Varicella (Chicken pox)

Varilrix®, Varivax®

If you had the illness chicken pox as a child (or adult) you do not need vaccination (no written proof needed).

If you never had it you will need a vaccination, or, if you prefer, you can have a blood test first to see if you are immune. Ask your doctor for a prescription.

You will not be required to take the Varicella vaccine if you are pregnant as it is a live vaccine.

Yearly seasonal influenza vaccine

Vaxigriptetra, Fluarixtetra, Influvac Tetra

Required if visa appointment is during the flu season (beginning of October to the beginning of February. Check with your local pharmacist to see if it is available at the time of your medical visit.

Pneumococcal

Prevenar 13

Required from the age of 65 onwards. For those who have never had the vaccination, the American Center for Disease Control (CDC) recommend an initial dose of P13

Download and print this file (PDF) and bring it along to your medical visit completed. If your family doctor fills out the check-list then he/she must mark the date each vaccine was given and countersign each vaccine.

ALL vaccinations on our list relevant to your age are necessary to obtain your US visa. They may differ from the French vaccination schedule but are obligatory for entry into the United States. 


Frequently asked questions

Who should carry out my vaccinations?

Please see your family doctor in advance to obtain required vaccinations. Your child can see a paediatrician. If you book your medical at least two weeks in advance, we consider that you have time to see your doctor.  .

What can I bring to the medical visit as proof of my vaccinations?

You can bring your carnet de santé (if you have one) or signed vaccination certificates/cards of all previously completed vaccinations. A signed letter from your family doctor listing your vaccinations and the dates they were given will also be accepted. Each vaccination must be dated and signed individually by the doctor. You can also bring blood test results to prove your immunity (for measles, mumps and rubella). Your doctor can prescribe the blood tests but bear in mind that the results make take at least a week to arrive.

What if I arrive at the medical visit and have not completed my vaccinations, nor written to you in advance asking for a prescription?

If you are not up to date with your vaccinations in line with CDC guidelines above on the day of your appointment with Dr Jolly or Dr Gunoy, you will need to make and pay for a new appointment to receive relevant vaccinations. Bear in mind this second appointment may not be available before your USA Embassy appointment.

What if I refuse certain vaccinations?

Dr Jolly and Dr Gunoy will not oblige you to take certain vaccines if you are opposed to them on moral or religious grounds, but you will have to contact the U.S. Citizenship and Immigration Services (see website www.uscis.gov for waiver information). Applicants must be vaccinated to date in line with the American CDC requirements for visa applicants.

However, Dr Jolly and Dr Gunoy will not oblige you to take certain vaccines if you are opposed to them on medical, moral or religious grounds, but it will be noted in your medical file and you must contact the U.S. Citizenship and Immigration Services website yourself for waiver information: www.uscis.gov.

You will also find examples of current or previous trade names of vaccines, but others exist and are accepted (for example, vaccines from countries outside France)
